Those options pertain to the Target and Focus frames as defined by he Heal Groups frame.
If you tick the Target and Focus frame on in - healbot options Skins - Frames - Heal groups... Then
Under healbot options Skins - Frames - Bars - Select Frame = Focus or Target - the options will become available.

I cannnot see anything you have set wrong and no indication of why Healbot is doing this.
I can only guess that something is causing interference to the running of Healbot. Do you have some frame moving addon that might be affecting the Healbot frame or another addon causing interference/giving errors.?
I can only suggest you disable all other addons and see if Healbot still does not work correctly.

Can we get a couple of screenshots please.
Shot of - Healbot Options - Skins - general - set it to Use skin for raid and take a shot please.
Also Healbot options - Skins - Frames - Heal Groups (set to use skin for raid up the top)
Also Healbot Options - Skins - Frames - Bars ( set the Visibility button on, then take a screen shot)
See If we can see something that might cause the problem.
